UPS has officially retired its remaining McDonnell Douglas MD-11 freighters, bringing an end to the company’s operations with the three-engine aircraft following a fatal accident in November and the subsequent grounding of the fleet.

NTSB says a catastrophic structural failure caused a UPS cargo jet to crash moments after takeoff from Louisville on November 4, killing all three crew members and eleven people on the ground. The findings come from a preliminary report released by the National Transportation Safety Board, which describes a violent separation of the aircraft's left engine and pylon assembly just as the MD-11F began to lift off. The accident also injured twenty-three people on the ground.

A UPS cargo aircraft has crashed near Louisville International Airport (KSDF) while operating as flight 5X2976 from Louisville, Kentucky, to Honolulu, Hawaii.
